Every work item is reviewed before it counts as done. The review is matched to the risk of the change: the riskier it is, the higher the bar it has to clear, and the more likely a person sees it.

What review decides

Each review ends in one of three outcomes:

Risk levels

Each work item has a risk level when it’s planned: Low, Standard, High or Critical. The risk level sets two things. How sure the review has to be. Iron Octopus approves a change on its own only when the review is confident enough for its risk level. Below that, a person reviews it. How many times it can go back. If a work item still has issues after its rework rounds, it goes to a person instead of looping. A person also reviews any acceptance criterion Iron Octopus can’t verify, can’t check yet, or judges not to apply.
These are the default rules. You can change the thresholds and limits for one project or your whole workspace.

Caught in review

Home has a Caught in review section listing what review found and sent back in the last 14 days: the issue, the feature and work item, its risk level, and the attempt it was approved on. Below it, a line totals review all time: how many changes were approved, sent back for rework, and sent to a person. It’s the plainest record of Iron Octopus checking its own work.
